Note:

Prior permission required for returns -- within 3 months and in
saleable condition with a charge of 15% of the invoiced value
assessed. Purchase Order Number required if not pre-paid and
mail or fax a copy of this Purchase Order to DEAFinitely Yours
Studio; net 30 days on sales, 2% monthly interest thereafter on
past-due balances. All payments must be payable in U.S. dollars
through a U.S. bank. DEAFinitely Yours Studio is not responsible
for any transactions transmitted through the Internet from this Order
Form. If you have any concerns regarding any transactions, contact
DEAFinitely Yours Studio.

          DEAFinitely Yours Studio reserves the right to change prices,
          products and/ or service without any prior notice.

For Maryland Residents Only:

Add 5% Sales Tax to the sub-total of your product selections.

Shipping/Handling for USA and Canada:

Add $5.00 if $50.00 or less
$8.50 ..........$50.00-$99.99
$12.50 ........$100.00-$150.00

Shipping/Handling for Overseas and other countries:

Add $14.00 if $50.00 or less
$17.50 .......... $50.00-$99.99
$20.00 .......... $100.00-$150.00

Payment by Mail:

Checks and money orders payable in U.S. dollars and drawable
on a U.S. bank. Feel free to print out this Printable Order Form and mail it along with the check or money order to DEAFinitely Yours Studio at the address given below.

 

Payment by Credit Card:

For security reasons, you may print out this Order Form and
once completed, fax it to 301-593-0700.

 

For Purchase Orders Only:

Original Purchase Orders must be faxed to 301-593-0700 after
submitting this Order Form. Mailing is also acceptable -- allow
up to 6 weeks for delivery if by sent in by mail.
